Where we work

Veteran-owned leadership
Founded by Markus Mastrud, RN, BSN, North Star is a veteran-owned, RN-led Oregon practice. Veteran ownership informs how we show up: prepared, accountable, calm and respectful of every person who trusts us with care.
After a career in military service and private security, I became a nurse to continue serving others in a different way. North Star Nurse Delegation was built on that same commitment—lead with integrity, teach with purpose, put safety first, and make every client feel like they are our only client.
Care provided
Care may begin with an assessment and medication review, then continue with teaching, delegation, monitoring and follow-through tailored to the client and care team.
We begin by understanding the client, their care team and the support that may be needed.
A registered nurse assesses the client and care plan before deciding whether delegation is appropriate.
The nurse reviews what is prescribed, what is actually taken and what a caregiver is being asked to give.
The nurse decides what may be delegated, to whom and under what circumstances.
Teaching is client-specific, and competence is observed rather than assumed.
Delegation is a continuing nursing relationship with follow-through as the client’s needs change.
Care changes, so nursing direction and delegated tasks are reviewed to keep the plan current.
Guidance is built around the client, procedure and caregiver rather than a generic task list.
When needs change, the nurse reassesses, communicates next steps and updates the care plan as appropriate.
Care and access
We support clients, caregivers, families, case managers and community partners in homes and eligible community-based settings.
We serve clients with Medicaid and private-pay arrangements. Every referral is reviewed for clinical fit, authorization and payer requirements.
Referral questions
Care starts with a clear conversation. Talk with our team about the nursing support your setting needs and the referral or payment path for the client.
A registered nurse assesses the client, decides which nursing tasks may be safely delegated, teaches the client’s caregivers to perform them, validates that competence in person and keeps monitoring as needs change. It is a continuing nursing relationship, not a one-time sign-off.
No. Delegation works through the caregivers already in place — the nurse teaches, validates and oversees them for the client’s specific tasks. Caregivers, families and case managers are colleagues in the same care.
In homes and eligible community-based settings across Multnomah, Washington and Clackamas counties in the Portland metropolitan area.
Clients, families, caregivers, case managers, agencies and clinicians can call the referral line to discuss whether the requested nursing support is a fit.
We will talk through the requested nursing support, setting, care team and next appropriate step. Every referral is reviewed for clinical fit, authorization and payer requirements.
It helps to know the client’s care setting, who provides their care today and whether the client has Medicaid or a private-pay arrangement — but you do not need everything answered to start the conversation.
